Special Circumstance Request Form

Please complete this form if you have extenuating circumstances that you believe should be considered in addition to the data provided on your Free Application for Federal Student Aid (FAFSA).

Estimated Cost Worksheet

Use your Official USF Award Letter to determine your out-of-pocket expense for tuition, room and board. Use this form to help determine the amount to budget on the FACTS payment plan.

FERPA Release Form (Federal Education Rights and Privacy Act)

Complete the FERPA Release Form to allow parents or other designated individuals the ability to discuss your financial aid and student billing information. Mail the completed form to the USF Financial Aid Office.

Employer Reimbursement

If you are receiving tuition reimbursement or tuition assistance from your employer, you are required to complete documentation each semester.

VA Benefit Information Form

Complete the Benefits Information Form to notify the Financial Aid Office of your expected VA Benefits.

Endowed Scholarship Application

Complete the Endowed Application to apply for or to renew an Endowed Scholarship. In order to be considered new students must submit their application by January 1st and returning students by March 1st.

Loan Increase Request Form

Complete this form if you would like to increase a loan that had previously been either reduced or declined

Loan Reduction Request Form

Complete this form if you would like to cancel part of your loan. Return the completed form to the Financial Aid Office, in order for any changes to be made.

Summer School Financial Aid Request Form

Complete the Summer School Financial Aid Request Form to be considered for summer school aid.

Bradfield/Avera Scholarship

These scholarships are offered to full-time or part-time Avera employees and their dependent children and spouses who are enrolled in a degree-seeking program at the University of Sioux Falls. Verification must be completed each year.

Outside Scholarship

All students are required to notify the University of Sioux Falls of any Outside Scholarships or Other Resources or sign that they will not be receiving any outside awards. Students receiving outside scholarships must attach scholarship documentation (such as letter of congratulation, certificates, etc.)
